Endoscopic Sinus Surgery for Treatment of Antrochoanal Polyp

|
Simple avulsion, endoscopic removal, Caldwell-Luc operation, and osteoplastic maxillary sinus operation were advocated as the treatment modalities for antrochoanal polyp (ACP). The choice of surgical method is important because of high recurrence rate following simple avulsion and high morbidity caused by radical procedure. We retrospectively reviewed twelve patients with ACP who underwent endoscopic surgery between Feb. 1, 1992 and Feb. 28, 1995. All cases were unilateral, and the most common symptom was nasal obstruction (100%). Nasal endoscopy and ostiomeatal unit computed tomography (OMU CT) were helpful for accurate diagnosis. Allergy skin tests with five common allergen extract were negative in all cases. Endoscopic simple avulsion was performed in three cases (25%), and polypectomy and FESS (functional endoscopic sinus surgery) was performed in nine cases (75%). One case out of three patients who had underwent endoscopic simple avulsion and two cases out of nine patients with polypectomy and FESS recurred. There were no immediate or late complications. These results suggest that the endoscopic surgery could be considered as one of the methods for the treatment of antrochoanal polyp.
